Your registration will be completed after we have received all your forms and the refundable deposit.

Participant Registration Form
This should be filled out once by all skiers at the beginning of the ski season, as part of the registration process for an event.
This form includes:
-
contact information
-
history with adaptive sports
-
seasonal ski goals
-
medical information to be used in the event of an emergency.
Any changes to information can be submitted through the office. Participants of the winter program and summer program have separate registrations forms that can be found under the 'Registration' tab.

Waiver: (Available at events)
This should be filled out by all skiers at the beginning of each snow and water ski season. A parent, guardian, or caregiver must sign for participants under the age of 18 and/or participants who are unable to read and comprehend the inherent risks described on the form.
Original signed waivers are needed for our records so please bring them to the event. Waivers will also be available at events.
You will not be permitted on the dock or in the water without a properly signed waiver.

Submitting a Refundable Security Deposit
The $30 refundable registration security deposit can be submitted via check, Venmo, or PayPal. This deposit can be refunded when you arrive at an event or kept as a donation to LOF. We do not charge individuals for participating in events. This $30 is only to secure your spot so that we can have as short of a waiting list as possible.
